Applications for Funding Requests

Applications for funding requests should be submitted to the Westport Lions by April 1st for spring disbursements and October 1st for fall disbursements.

Instructions for completing form:

  1. Click on button to open form
  2. In your web browser choose Save As to your desktop
  3. Open and complete form
  4. Save
  5. Email as attachment to westportontariolions@gmail.com
